The warhead shouldn't arm at that distance, either it was faulty or it had been tampered with. Or perhaps some propellant gasses came out the side instead of the rear.

My theory is he got sniped right at the time he fired. Look closely, his face was intact but the backside of the head blown out.
He say there poking his head out aiming and taking his time then chatted with his comrades and went back to aiming then.......
No wonder.
